# Add based on selected value from a field selection

My Mautic version is: v5.0.4  
My PHP version is: 8.1.28  
My Database type and version is: 10.6.17-MariaDB-cll-lve-log  
Webhost: Name Cheap  
Softaculous

**Your problem**

My problem is:  
I created a form and several questions requuire additional information

see the picture. as you can see answers 3&4 should have produced other additial details however this feature isnt working Please advise.

These errors are showing in the log:  
not errors

Steps I have tried to fix the problem:  
unknown steps to take need avice
