# Form Submission 500 Error

Hey everyone,  
  
  
  
I’m slowly replacing the existing forms that were on my site with Mautic forms, but on submission, the url “[http://navarrodentalgroup.com/mautic/form/submit?formId=1](http://navarrodentalgroup.com/mautic/form/submit?formId=1)” is bouncing to a 500 internal server error. I don’t have the form set to redirect to a page, it is supposed to just bounce back to the form itself. However, Mautic is creating/updating the lead based off of an email address, sending the form results to the designated address, and sending a confirmation email to the “client” when they fill out the form. I’ve tried changing the permissions on the /mautic/ subdirectory but haven’t had any success with that.

Hi Conor,

Are you able to have a look at the /path/to/mautic/app/logs/mautic\_prod.php log file and see if is the same as the one as I"m finding here [https://github.com/mautic/mautic/issues/380](https://github.com/mautic/mautic/issues/380) . If it is could you add your steps and error log to the GitHub issue as well.
