**Your software** My Mautic version is: 4.0.1-12-r04 My PHP version is: 7.4.25 My Database type and version is: Ver 15.1 Distrib 10.6.4-MariaDB, for Linux (x86\_64) **Your problem** My problem is: Whenever I attempt to save a form build, I get the 500 Internal Server Error message, These errors are showing in the log: [proxy\_fcgi:error] [pid 16361:tid 140534525937408] [client 62.253.3.70:52240] AH01071: Got error ‘PHP message: PHP Notice: Undefined index: ct in /opt/bit nami/mautic/app/bundles/PageBundle/Controller/PublicController.php on line 456’ Steps I have tried to fix the problem: None as yet ---

Hi, this is a know bug. It happens because you disabled style in the form settings. You will need to change it back for now. Since you get an error, you will need to do ot in the db directly. Find the forma table and change the style column content to 1.
