I am a total newbie and am trying to create form with either a drop down or radio buttons but it just won't work

Hi guys
I am a total newbie to Mautic and I am trying to create a form to send to brides asking them questions about their wedding to use as the basis for a quote.

I am trying to put either a dropdown checklist or radio buttons to tick. I just can’t work out what to put in the contact field or the properties.

Can anyone help a girl out please.

Windows 10
My Mautic version is: 2.15.1
My PHP version is:7.0.3

Your problem
My problem is: I can’t work how to create the form fields eg either radio group, or dropdown checklist for my form.

These errors are showing in the log:

Steps I have tried to fix the problem: by reading what I can, and viewing videos. I am a total newbie. thank you for your helo.

there are slect type data type in the form you can try
