show field when checkbox selected

I am having difficulties - probably simple but just missing it.



I have a form with a checkbox with 3 values. Only one of the three maybe selected. But based on the selection I need an additional field to show based on selection.

I need to show a simple text area field for when either of the 3 is checked.



For example:

If Selection 1 ticked

Text area shows up with label please enter your relevant serial number related to this product- with text area

or Selection 2 ticked

Text area shows up with label please enter your relevant batch number related to this product - with text area



Can’t seem to figure out how to do this Please help

@dellabella Did you ever find a solution for Dynamic Fields? I have the same requirement.

@tpizzamiglio i think you can accomplish that using jquery
