Uncaught SyntaxError: Unexpected number

I created a focus item that calls a form. This form will redirect to a URL. The focus item works as expect, the form writes lead info but it never redirects. Stay saying “please wait” in the form button. I saw browser console log and found the following error: - Uncaught SyntaxError: Unexpected number



I am using Mautic 2.11 version. Any help will be apreciated.
