We just ran a survey for a client using Mautic Forms.
We did a very nice trick on keeping the survey “anonymous” but used page tracking to send a thank you mailer once the survey was completed.
Also used multiple pages with the page break.
Nice thing is you can obviously get all the results from the form results or by creating custom report.
