I tried going to builtin.mautic.net/form/generate.js?id=9 and it came up blank; if I go to an equivalent link for one of my forms it comes up full of code. Maybe check your form is not blank?

You have to embed both the javascript and the html form fields. Otherwise there will be no form to show.
