Lead going to two lists--only supposed to be one

I have a form and the action on that form is to place the lead on a particular list. It is doing that but it is also putting the lead onto an entirely different list as well–there is no such action on the form. Not even really sure where to start looking for the problem.

Any help on this? this is creating a problem because emails are being delivered to people who are not supposed to be on the second list and they are completely unrelated to what they signed up for. Thanks.
