Campaign action not triggering

Hi, need help with campaign actions not triggering. Sharing a screenshot of a test campaign I've set up: http://prntscr.com/bmxs3n. Here's what happened.

1. Intro email sent okay as scheduled.
2. Points deducted although the email was not only read but a reply was sent as well.
3. Points not added for opening email.
4. Contact successfully assigned to a new segment.
5. Follow up email not sent. This is set to be triggered by opening the intro email.
