My Mautic version is: 2.15 (I think)

Your problem
My problem is: We have a contact that, according to Mautic the contact was “identified by Quick Pop-up Form”, a Mautic-built contact form on our site. There are also records of page hits, accessed from IP, and Contact identified by source. However, when we look at that form, there is no record of this person having filled out the form. Are we missing something?
These errors are showing in the log: No errors
Steps I have tried to fix the problem: Just starting to research this issue.
