Form submission causing actions to trigger twice?

I have a situation where a simple form submission is triggering all the resultant actions twice. Emails sent twice, points added twice, pushed to suitecrm twice and so on.



It is a simple stand alone form.



Where / how do I start to trouble shoot this?
