Form Submission Not Triggering Email

Hey everyone,



I’ve been running this campaign lately: . Once a form is submitted, the campaign is supposed to send a follow up email with a link to an informational whitepaper the contacts filled out the form for, but it does not. It is also supposed to adjust the contacts points by adding 5, which it does not do either. Any ideas as to why this is not working correctly?
