Is it possible to set a tag to a contact by the multi-select field the lead has chosen on a mautic form?

You could do it via a campaign. But you would need to check for each possible value.

Other than that, possibly a custom java script could read the values from the form and send them as parameters with an event: Contact tracking | Mautic
