Wrong contacts preference center page

My Mautic version is: 4.7
My PHP version is: 7.4.33
My Database type and version is: 10.6.11-MariaDB

Your problem
My problem is: When a user clicks on a unsubscribe link it shows the wrong contacts preference center page, The activity tracker shows the correct user interacting with the email but the wrong user preference center and unsubs the wrong contact

These errors are showing in the log: No Errors

Steps I have tried to fix the problem: Not sure where to start? It only happens on one of our instances

do you have a default preference page setup? You can also set individual forms to use a preference page of your choosing.

I am using a Custom Page as a Preference Center because I needed a specific look. We wanted a single button click to unsubscribe. so I am using javascript to handle the button click and toggle the check mark and submit the form. it works on 5 other instances of mautic but 1 of them shows the wrong username in the page even before clicking the button
